COMMUNITY HEALTH NEEDS ASSESSMENT

A Community Health Needs Assessment is a report that assists the hospital’s governing body in complying with the mandates and spirit of the Affordable Care Act. It identifies healthcare needs, establishes priorities based on them, and implements a strategic plan around the resources required to meet them.

Based on the CHNA's findings, a three-year implementation strategy for CVMC that addresses the community’s health needs will be developed and monitored continuously.
