Welcome to the Coosa Valley Medical Center Transitional Care
Unit. We are a short-term skilled unit designed to provide both
your medical and skilled rehabilitation needs. We provide comprehensive
care including, but not limited to, IV Therapies, Physical Therapy,
Occupational Therapy, Speech Therapy, wound care, nutritional
management and specialized activities. We treat each patient
with an interdisciplinary approach paying special attention
to their medical, mental, emotional, physical, and spiritual
needs. Our 25 bed unit is designed for short-term stays, offering
both private and semi-private patient rooms, Activities room,
and a Rehab area that is well equipped to meet an individualized
plan of care for you or your loved one. We welcome referrals
from any acute care facility and will be more than happy to
work hand in hand with them to provide your short-term needs.
All admission criteria must be meet including, but not limited
too, an acute care admission of at least 3-days occurring within
the 30 days prior to your admission, and is certified as medically
necessary by your physician.
Our unit offers 24-hour medical care. Our team is dedicated
to providing our patients with the highest quality of care,
as well as help you and your family transition back into your
home setting. We complete your Transitional Care experience
at discharge by arranging for any equipment needs, Home Care,
Hospice Care, or other needs as ordered by your physician to
assist you.
