The Case Management Department at Coosa Valley Medical Center is here to assist the patient when continued care is needed. Through the process of discharge planning assessment, the patient's need for home health, hospice, nursing home, rehabilitation placement, or assisted living placement can be determined. Additional services such as drug and alcohol rehabilitation and access to durable medical equipment for home use can be also determined and recommended. The department works closely with other agencies to help provide assistance in order for patients to receive an optimal level of care.
