HEALTH PLAN
Coosa Valley Medical Center provides employees and their eligible dependents with financial protection against the high cost associated with care for serious illness and injuries. Employees must be a full-time or benefit eligible part-time employee working a minimum of 30 hours a week or 60 hours per pay period to be eligible.
Coosa Valley Medical Center picks up the major portion of the costs to provide our employees with health coverage. The bi-weekly cost is deducted from the employees’ paycheck 24 times a year on a pre-tax basis. Your actual cost will depend on the amount of people covered under your plan. The current rates are as listed below;

DENTAL PLAN
Coosa Valley Medical Center provides employees and their eligible dependents with the option of a dental plan. Employees must be a full-time or benefit eligible part-time employee working a minimum of 30 hours a week or 60 hours per pay period to be eligible.

THE GROUP LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT INSURANCE PLANS
Employee Coverage
Coosa Valley Medical Center provides a company-paid Basic Life and Accidental Death and Dismemberment benefit equal to one times the employees’ annual salary* (rounded up to the next highest $1,000 / $500,000 max) free to all regular full-time and part-time benefit eligible employee working a minimum of 30 hours per week or 60 hours per pay period to be eligible for this benefit.
*Annual base salary excludes any overtime, bonuses, shift differential, or other special compensation.
Dependent Life Benefit
The Life Insurance Plan provided to employees by CVMC also includes a company-paid Dependent benefit in the amount of $3,000 for the employee’s spouse and $1,000 for each of the employee’s children. Generally, dependents include the employee’s legal spouse and children between the ages of six months and 19 years.
SUPPLEMENTAL LIFE
Regular full-time and benefit eligible part-time employees have the opportunity to purchase Supplemental Term Life coverage for self, spouse, and dependents. Premiums are at affordable rates and will be payroll deducted. Plan(s) are portable.
SHORT TERM DISABILITY
Coosa Valley Medical Center provides full-time and benefit eligible part-time employees with a Short Term Disability plan that can continue part of their income if they are unable to work because of illness or injury. Plan pays 60% of the employee’s base salary* up to $750 weekly. The policy is 100% employer paid.

LONG TERM DISABILITY
Coosa Valley Medical Center provides full-time and benefit eligible part-time employees with a Long Term Disability plan that can continue part of their income if they are unable to work because of illness or injury. Plan pays 40% of the employee’s base salary* up to $5000 monthly. The policy is 100% employer paid.
Eligible employees have the ability to purchase additional Long Term Disability that pays an additional 20%of base salary*.

MEDICAL AND DEPENDENT CARE SPENDING ACCOUNTS
Employees of Coosa Valley Medical Center are able to enroll in Medical and Dependent Care Spending Accounts. These accounts are designed to let employees take advantage of non-taxable benefits. By using these reimbursement accounts, you can use before-tax dollars to pay for:
- certain health care expenses not covered by the health and/or dental plans, and
- certain dependent care expenses.
Employees must be full-time or benefit eligible part-time working a minimum of 30 hours a week or 60 hours per pay period to be eligible to participate in the spending accounts.
RETIREMENT PLAN
Coosa Valley Medical Center makes available for all full-time benefit eligible employees a retirement plan (403b). Employees are able to contribute to this plan immediately. Employees may also be able to roll over funds from an existing qualified retirement plan, which would allow them to consolidate their assets. Upon eligibility for the plan Coosa Valley Medical Center will began contributing the equivalent of 1% of an employee’s base salary each pay period into a 403b account on their behalf. Once the employee begins participating in the retirement plan, Coosa Valley Medical Center will match 50% for the first 4% and then a 100% match for the next 2%. This plan does allow for catch-up contributions for those employees age 50 or over.
The money employees contribute to their retirement plan account is always 100% vested. However, the amount of money contributed to the employee’s account by Coosa Valley Medical Center on behalf of the employee will become vested based on the following vesting schedule;

*For a year of service to qualify for vesting purposes, you must have worked 1000 hours.
Coosa Valley Medical Center also offers payroll deductions for after-tax Mutual Funds and a Section 529 Plan for college savings.
EARNED TIME OFF (ETO)
Earned Time Off (ETO) is a time off plan for all regular full-time employee. It combines vacation, holiday and sick leave into one flexible plan. Employees begin to accrue ETO after 90 days of employment. Once the employee successfully completes the initial employment period (typically six months) they are then eligible to use their ETO with supervisor approval. After one year of employment employees become vested in their ETO bank.
EDUCATION ASSISTANCE
All full-time and part-time benefit eligible employees who have successfully completed the initial six (6) month employment period are eligible for “Education Assistance”. Covered expenses include tuition, books, and fees up to $3,000 per calendar year for full-time and $1,500 per calendar year for part-time employees.
The degree that an employee pursues should pertain to a position somewhere in the organization. In addition to reimbursement for successful completion of courses from an accredited college or university, employees can receive reimbursement for (a) non-credit courses related to advancement at Coosa Valley Medical Center, provided those courses do not duplicate courses offered by Coosa Valley Medical Center, (b) professional certification/licensure when examinations in professional fields are related to advancement within Coosa Valley Medical Center, (c) college level examination program (CLEP), SAT, ACT, GRE or GMAT.
EDUCATION STARTER LOANS
Many employees are interested in furthering their education, but have difficulty coming up with the initial cost of tuition, books and fees for the first course. Coosa Valley Medical Center offers a program that tries to help with this problem. “Education Starter Loans” are available to all full-time and part-time benefit eligible employees (must complete initial 6 months of employment).
EMPLOYEE ASSISTANCE PROGRAM (EAP)
When employees have a personal problem, their home life suffers as well as their performance at work. The Employee Assistance Program (EAP) can give employees a highly confidential source of help. All employees of Coosa Valley Medical Center have access to EAP. The company pays the total cost of this program for all employees and their dependents.

FITNESS & WELLNESS
The Coosa Valley Fitness & Wellness Center provides a discount plan for all employees of CVMC and their family members. The Center has a variety of activities including aerobics, racquetball, free weights, and cardio equipment.
METLIFE
Benefit eligible employees of Coosa Valley Medical Center are able to insure their automobile, home, and other personal property at special cost group rates. Premiums are conveniently payroll deducted.
U.S. SAVINGS BOND PAYROLL DEDUCTION PLAN
Coosa Valley Medical Center offers benefit eligible employees an easy and convenient way to purchase U.S. Savings Bonds.
PAYROLL DIRECT DEPOSIT
Employees of Coosa Valley Medical Center have the option to have their paycheck directly deposited in, up to three (3) individual accounts at no charge to the employee.

Parking is free to all employees of Coosa Valley Medical Center.
